Attention all active and retired "A" card members.

A new revision to Form 124 was issued by the Internation Office to designate beneficiaries. This new revision to Form 124 is the only one that will be accepted by the IO.

It is preferred that the form be filled out using the online tool. If the form is printed and filled out manually, you must use black ink and entries must be legible.
